In partnership with The Lions Club, 2-1-1 provides assistance for eye glasses and, in some cases, eye exams.
Qualifications for eye glasses are: Fairfield County residency for at least six months, proof of income for everyone in the household, social security cards for everyone in the household, proof of address, and eye prescription if applicable.

Required
- Proof of income for everyone in the household
- Proof of address
- Valid ID & Social Security Card for all household members
For a glasses-only application, also include:
- Prescription for the eye glasses

Once you have completed the application and provided Fairfield County 2-1-1 with all necessary documents, your application will be reviewed and sent to the Lions Club for final approval. This process takes 4–6 weeks. When Fairfield County 2-1-1 receives an answer concerning your application, you will be contacted by telephone.
