In partnership with The Lions Club, 2-1-1 provides assistance for eye glasses and in some cases, eye exams.

Qualifications for eye glasses include Fairfield County residency for at least six months, proof of income, social security cards for everyone in the household, proof of address and eye prescription if applicable.

Lancaster Lions Club Application Instructions

*Fill out both pages of the application completely.

Return the completed application to Fairfield County 2-1-1 (108 West Main Street, Lancaster, OH 43130) with the following documentation:

· Proof of income for everyone in the household

· Proof of address

· Valid ID & Social Security Card for all household members

For Glasses only Application, also include

· Prescription for the eye glasses

 

*Note: Your application can NOT be processed without ALL of this information.

 

Once you have completed the application and provide Fairfield County 2-1-1 with all the necessary documents, your caseworker will review the application and submit it to the Lions Club for final approval. This process will take 1-2 weeks. When the caseworker receives an answer concerning your application, you will be contacted by telephone.
